Parent Agreement & Waivers

Below are the full legal documents of My World Shaporenko (My World PROSTIR): the Parent Agreement, Program Policies, Assumption of Risk, Liability Waiver and Release, and the Media Release. By accepting the terms during registration you agree to these exact documents.

The English version of these documents is the legally binding one — they are governed by the laws of the State of California (USA).

Parent Agreement, Program Policies, Assumption of Risk, Liability Waiver and Release

This Agreement is entered into by and between My World Shaporenko (“My World”) and the Parent/Guardian on behalf of the participating child.

Program Description

My World provides educational, enrichment, creative, cultural, technology, and arts-based activities for children ages 5–13, including but not limited to:

  • Art, Painting, Drawing
  • Clay and ceramics
  • Crafts
  • STEAM activities
  • AI & Technology activities
  • Cultural education
  • Etiquette workshops
  • Drama and creative expression
  • Group projects, demonstrations, presentations
  • Seasonal workshops and special events

Program activities may vary throughout the year.

Program Locations

The primary location is: 101 Creekside Ridge Ct, Roseville, CA 95678. Program locations may change from time to time. Parents/Guardians will be notified in advance of any location changes through email, text message, or other official communication methods.

Registration and Enrollment

Participation in My World programs requires:

  • Completed Registration Form
  • Emergency Medical Authorization
  • Parent Agreement and Waiver
  • Payment of applicable fees

Enrollment is not guaranteed until all required forms and payments have been received.

Payment Policy

Program fees must be paid according to the payment schedule communicated by My World. Fees help reserve a child's place in a class or activity. Returned payments, declined transactions, or unpaid balances may result in suspension of participation until the account is brought current.

Attendance, Absences, and Make-Up Classes

Parents are encouraged to notify My World as soon as possible if a child will miss a scheduled class. When advance notice is provided, My World may offer a make-up opportunity in another age group or class when appropriate and when space is available. Make-up classes are not guaranteed and remain at the discretion of My World. No cash refunds are provided for missed classes. Credits may be issued for future participation when approved by My World.

Illness Policy

Children who are experiencing symptoms of illness, fever, vomiting, diarrhea, contagious conditions, or other symptoms that may affect the safety and well-being of participants should remain home. My World reserves the right to request that an ill child be picked up if symptoms develop during a program.

Pick-Up Policy

Parents/Guardians are responsible for ensuring that children are picked up promptly at the conclusion of each program. Children will only be released to Parents/Guardians, authorized individuals listed on registration documents, or individuals specifically approved by a Parent/Guardian in advance. My World reserves the right to request identification before releasing a child. Children will not be released to unauthorized individuals.

Late Pick-Up

Families are expected to pick up children promptly at the end of each program. A grace period of up to fifteen (15) minutes may be provided. Repeated late pickups may result in additional fees, restrictions on participation, or other reasonable measures necessary to support program operations and staff scheduling.

Behavior Expectations

My World is committed to providing a safe, respectful, and positive environment. Children are expected to:

  • Treat others respectfully
  • Follow instructor directions
  • Use materials appropriately
  • Refrain from bullying, harassment, threats, or disruptive behavior
  • Respect program property and equipment

Parents acknowledge that My World may address behavior concerns through redirection, parent communication, temporary removal from an activity, or, when necessary, dismissal from a program.

Safety and Supervision

Reasonable supervision will be provided during scheduled program hours. Parents remain responsible for accurate emergency information, transportation to and from programs, medical needs and medications, and timely pick-up.

Field Trips and Off-Site Events

My World may occasionally offer optional field trips, museum visits, performances, educational experiences, or special events. Children will not be transported by My World; parents are responsible for transportation unless otherwise communicated. Certain events may require parent attendance. Separate waivers, permissions, and event-specific forms may be required.

Assumption of Risk

Parent/Guardian understands that participation in children's educational, creative, technology-based, cultural, and recreational activities involves inherent risks. Such risks may include, but are not limited to:

  • Slips, trips, and falls
  • Minor cuts, scratches, or bruises
  • Contact with art materials, clay, paint, glue, markers, and craft supplies
  • Use of scissors and age-appropriate tools
  • Reactions to materials despite reasonable precautions
  • Equipment-related incidents
  • Group activity injuries
  • Technology-related equipment use
  • Accidental damage to clothing or personal belongings
  • Injuries resulting from normal child movement and participation

Parent/Guardian voluntarily accepts and assumes all ordinary and inherent risks associated with participation.

Release of Liability

To the fullest extent permitted by law, Parent/Guardian releases, waives, and discharges My World Shaporenko, its owner, instructors, independent contractors, assistants, volunteers, agents, and representatives from claims arising from ordinary negligence associated with participation in program activities. This release applies to claims involving personal injury, property damage, accidental loss, and participation-related incidents, except where prohibited by law. This Agreement does not release liability for gross negligence, willful misconduct, reckless conduct, or conduct that cannot legally be waived under applicable law.

Independent Contractors

Parent/Guardian acknowledges that My World may engage qualified independent contractors, instructors, artists, educators, presenters, or specialists to conduct classes, workshops, demonstrations, and educational activities. Such individuals are independent contractors and not employees of My World.

Photography and Social Media Acknowledgment

Parent/Guardian acknowledges that photography, video recording, and content creation are a regular part of My World programs and are governed by the Media Release consent given as part of enrollment (see below).

Changes to Information

Parent/Guardian agrees to promptly notify My World regarding changes to phone numbers, addresses, email addresses, medical information, insurance information, emergency contacts, and authorized pick-up persons.

Electronic Signatures

Electronic signatures and electronic acceptance (including checking an acceptance box in an online registration form) shall be considered equivalent to original signatures and shall be fully binding.

Severability

If any provision of this Agreement is determined to be unenforceable, the remaining provisions shall remain in full force and effect.

Governing Law

This Agreement shall be governed by and interpreted under the laws of the State of California.

Acknowledgment

By accepting these terms during registration, I acknowledge that I have read this Agreement in its entirety; I understand the Program Policies, the Assumption of Risk provisions, and the Liability Waiver and Release; I voluntarily agree to these terms on behalf of myself and my child; and I am the parent or legal guardian of the participating child (or an adult participant accepting for myself).

Media Release and Consent

I understand that photography, video recording, audio recording, live streaming, digital content creation, promotional materials, educational materials, artwork displays, social media content, website content, printed materials, newsletters, advertisements, and other media may be created during My World Shaporenko programs.

By granting media consent, I give permission for My World Shaporenko and its representatives to photograph, record, film, reproduce, publish, display, distribute, and use my child's (or my own) image, likeness, voice, artwork, projects, participation, and related content for educational, promotional, marketing, website, social media, print, and business purposes without compensation.

I understand that such materials may be used in current and future publications and may remain in use indefinitely.

I understand that My World Shaporenko will not intentionally publish sensitive personal information about my child without additional consent.

Media consent is optional: declining it does not affect enrollment or participation.
